Players aged 16–17 are welcome at Olympus Volleyball events and activities. A parent or legal guardian must complete and sign this form before participating. The form can be used for multiple Olympus Volleyball events and activities; fill in the optional event section only when a specific date or event needs to be identified. Players without a signed form on site will not be permitted to play.
Print, fill, sign, and bring to the event or activity.
What the form covers

Athlete accounts & public profiles
Players may optionally have an Olympus athlete account and a public profile (name, photo, positions, team and event history). For a minor, the parent or guardian decides — you can decline a public profile or request its removal at any time. Contact information is never shown publicly. Accounts use passwordless sign-in (email link or Google); no password is stored. Full details are in our Privacy & Data Policy.
